Discharge Resources for homeless patients, prepared by Julie Camba, class of 2015, for UMPH practicum:

  • Discharge Instruction Template:
    • You're ready to leave the hospital.  For breakfast (6:30 - 8:00 AM)  or dinner (4:45-5:45 PM), you can get a fresh, healthy meal at Miriam's Kitchen.  It is nearby GW hospital at 24th and G St.
    • Even if you don't normally stay at shelters, consider sleeping at one tonight or staying with a friend so that you can recover fully from your hospitalization.  The closest shelters to GW are:
      • Central Union Market at 65 Mass. Ave NW
      • Drop by the Georgetown Ministry Clubhouse open from 8 AM - 4 PM every day.  You can use computers, have a coffee/sandwich as well as do laundry or take a shower (first come, first serve).  Location is the alley entracnce of Grace Church 1041 Wisconsin Avenue, a block south of M street toward the river.
